Please visit https://surveys.panoramaed.com/nycdoe/login and enter your child’s OSIS number preceded by a lowercase “f.” This number can also be found on your child’s report card, Jupiter Ed account, or school ID. If you need help locating your child’s OSIS/ID number, please contact Parent Coordinator Vickiana Ramirez . You can use the hard copy we sent home in a green envelope last week.
Just bring it back to school and we will mail it.
