Instructions to obtain Employment Certificate/Working Papers, for minors (age 14 to 17 years old):
Complete the Application for Employment Certificate

Student should complete Part I

Parent/Guardian must sign Part I

Provide a completed Physical Fitness Certification from the student's primary doctor indicating that the student is physically cleared to work. Please note that PSAL forms and/or immunization records are NOT acceptable substitutes.

Provide a copy of one of the following as proof of Age: School ID, birth certificate, passport, or other official proof of age.
Employment Certificates are issued by Ms. Ramirez in the main office after school ONLY.
